Frequently Asked Questions About Rehabilitation

ASC’s Short-Term Rehabilitation program assists those who are impaired by acute or chronic illness or are recovering post-surgery, in order to regain the fullest possible independence. We strive to help participants return home safely with any adaptations or modifications they may need to remain at home.

ASC’s Short-Term Rehabilitation program provides physical, occupational, speech/language therapies and a full range of interdisciplinary support, including respiratory therapy services, doctors, nurses, social workers and other senior care professionals. Using skilled therapy interventions, medically integrated programs and holistic wellness services, we tailor a care plan to improve independence and return home.

Returning home quickly is always the goal. At ASC, we carefully balance the goal of returning home while achieving goals that will decrease the chance of patients experiencing a rehospitalization. When a patient is ready, a thorough home therapy assessment is completed in person or remotely to ensure safety and help the patient and caregivers gain confidence.

Our Road to Recovery meetings, held within the first 72 hours after admission, bring the interdisciplinary team, patient and family/caregivers together to identify goals needed to return home. There is discussion about how those goals may be best achieved, identifying possible obstacles that need to be overcome by adaptions, equipment, etc.

Treatment is provided based on the patient’s goals, expected outcomes and a physician’s order. The treatment plan is created from this meeting and depending on intensity, therapy will vary from 3-6 days per week.

Comfortable clothing and supportive footwear are recommended. Please refrain from bringing valuables or cash.

ASC’s Short-Term Rehabilitation wings (at selected locations) include private suites, exclusive dining area and courtyard, electric beds, nicely appointed living rooms, elegant family lounge area, cable TV and phones.

If covered by Medicare Part A, a therapy patient may be eligible to receive skilled nursing care after a qualifying hospital stay. However, there are some limits to the duration of Skilled Nursing Facility (SNF) stays and the circumstances under which this care can be billed. Medicare will fully cover up to 20 days of SNF care per hospital stay, and could partially cover an additional 80 days, provided one’s hospital stay meets some specific conditions.
